Preparing for your appointment

We recommend that you have your consultation 6-8 weeks prior to your departure date to ensure there is time to receive all vaccination doses that you will require. If you have last minute travel plans, it is also important to make an appointment so that we can prepare as adequately as possible.

Once you have booked an appointment, you will be asked to fill out a Pre-Travel Screening Form. This will take approximately 15 minutes. It can be emailed to you directly or found on our website. Completion of this form prior to your appointment will allow us to spend more or your appointment time discussing your health and wellbeing.

Please bring the following to your appointment:

  • A detailed itinerary for your trip including any layovers, airline stops, locations and accommodation types within your destination countries
  • Provincial Health Card
  • Third Party Health Insurance plan
  • Any records of your immunization history
  • Current medication list
